Case Management

Case management services are provided by case managers who work on-site, allowing for convenient access by clients, staff, and families. Case management services provides:

• Assistance in gaining access to needed Wyoming Home and Community Based Waiver and other Wyoming Medicaid State Plan services, as well as needed medical, social, educational, and other supports.
• Visititation by case managers in day programs, during therapy sessions, on the job, and in your home in order to ensure that your needs are being met.
• Availablity by clients, staff, and family members who can ask questions and discuss concerns, and have those concerns addressed in a timely manner.
